Medical Care: Group Health Medical Insurance
Employees have two different plans (including an exciting HSA option) to choose from with monthly premiums up to $43.00 for self coverage. Spouse and dependent(s) coverage is paid in full by the employee. All premiums are deducted from the employee’s paycheck on a pre-tax basis and are taken evenly from the Mid Month and End of Month paychecks to cover premiums for the next month.
Dental Care: Premera
Employee’s coverage is $5.00 per month. Spouse and dependent(s) premiums can be deducted on a pre-tax basis and are taken evenly from the Mid Month and End of Month paychecks to cover premiums for the next month.
Short-Term Disability
Standard benefits: $200 per week; pays after ten (10) days. No cost to employee.
Long-Term Disability
Standard benefits: 60% pay (10k monthly maximum) after 90 days. No cost to employee.
UNUM Life Insurance
Voluntary group term life insurance. Priced per $10,000 of coverage, with premiums depending on age, smoker/nonsmoker.

Employee receives birthday off, after one year of employment.
Not available the first six months of employment; accrue 2 weeks each year for the first five years of employment, accrue 3 weeks each year for fifth year of employment through ninth year of employment, and accrue 4 weeks each year after ten years of employment.
Eight (8) days per year. (Management level personnel do not accrue.)
Effective May 1, 2010, the firm contributes $50 per month toward the purchase of an ORCA card which can be used on any form of public transportation. The employee's expenses over and above this amount are deducted from the employee's pay on a pre-tax basis within IRS regulations. The firm no longer makes a contribution for parking.
$2,000 ($1,000 at commencement of referral employment; $1,000 after 6 months of satisfactory employment, if both employees are still with the company)
Dependant Care Reimbursement
$5,000 maximum per calendar year. Administered by Paychex, Inc.
Unreimbursed Medical Expenses
$5,000 maximum per plan year (January 1 - December 31). Have pre-tax withdrawals placed into this account for you. You can use the money to reimburse medical visit co-pays, prescription co-pays, and other medical expenses that are not covered by insurance. Administered by Paychex, Inc. (Not eligible for this benefit if you sign up for the HSA Medical Insurance.)
The 15th and End of Month; paid on Friday if the 15th or the last day of the month fall on a weekend

Maximum $16,500 contribution in 2010, $21,500 if you are over or turning 50 in 2010, with IRS limitations. Eligible after four (4) months of employment. (Firm does not make a contribution.)
New employees are eligible for the profit sharing plan at the first enrollment period after completing 2 years of employment. Enrollment periods are January 1 and July 1 each year. The Plan is totally funded by the firm and contributions are made annually at the discretion of the firm. If a contribution to the retirement plan is declared, it will be made after the completion of the calendar year in which a new employee becomes eligible. This contribution has historically been 10% of the employee’s salary from their enrollment period through the end of the calendar year. The Firm reserves the right to change the contribution rate or other terms of the Plan in the future,
i.e. hire date is October 11, 2008. The first eligible enrollment period is January 1, 2011. A percentage of the salary earned between January 1, 2011 and December 31, 2011, will be contributed into your pension plan on or about April 15, 2012. You will be 100% vested the moment that first contribution is made.
